Divinagracia Home Care is a licensed Level 6 Adult Residential Facility (ARF) serving adults with developmental and physical disabilities in the greater Sacramento area. We provide a safe, nurturing, and home-like environment where every client is treated as an individual — not a case number.

Our facility operates in close partnership with Alta Regional Center, adhering fully to California State Department of Social Services (CDSS) regulations and all Alta Regional service guidelines. We welcome both conserved and non-conserved clients, and embrace family and guardian involvement at every stage of care.

Our Services

Individualized Daily Care

Our trained caregivers and licensed nursing staff provide around-the-clock support tailored to each client's Individualized Program Plan (IPP). Services include personal hygiene and grooming assistance, medication administration and management, nutritious meal preparation, mobility support, and ongoing health monitoring.

Every care plan is developed collaboratively with Alta Regional coordinators, family members, and medical professionals to ensure each client receives care that respects their goals, preferences, and clinical needs.

Community, Activity & Belonging

Life at Divinagracia Home Care extends well beyond our walls. We actively assist clients in enrolling in Alta Regional-approved day programs and adaptive sports programs designed for adults with disabilities.

From structured day activity centers and vocational programs to wheelchair-accessible recreational leagues and Special Olympics opportunities, we connect each client with enriching community experiences that build skills, confidence, and lasting friendships throughout the Sacramento region.

Who We Serve

Every Client, Every Situation

We welcome non-conserved clients who direct their own care, as well as those whose affairs are overseen by a conservator, family guardian, or regional center coordinator. Our staff is experienced in working alongside legal representatives, healthcare teams, and Alta Regional to ensure seamless communication and transparent reporting.

Referrals are coordinated through Alta Regional Center. We are happy to guide families and coordinators through every step of the placement and intake process.

Registered Nurse (RN) • Owner & Facility Director • 25+ Years in Healthcare

Marylin Divinagracia is a licensed Registered Nurse with over 12 years of experience in healthcare, spanning acute care, community health, and residential care for adults with developmental and physical disabilities. Her clinical background provides Divinagracia Home Care with a level of medical oversight, regulatory knowledge, and professional rigor that distinguishes the facility.

Driven by a deep personal commitment to dignified, individualized care, Marylin founded Divinagracia Home Care to create a facility that bridges genuine clinical expertise with real human warmth. She oversees all aspects of client care coordination, staff training, CDSS regulatory compliance, and family communication — ensuring every person in her home is truly seen, heard, and cared for.

Marylin works directly with Alta Regional Center coordinators, clients' medical teams, and families to uphold the highest standards of care and to advocate for each client's well-being and quality of life.
